Though definitions change over time. In the past, this was interpreted to mean, for example, someone who dresses and acts the part of a different gender than their birth sex, but does not have gender confirmation surgery (aka a sex change), at which point they become transsexual. So, depending on who you're dealing with they may be using that definition.
